Employee wellbeing self-check: nine planning questions
Already have activities or want a structured start? Answer nine organizational questions to see unresolved points and suggestions for your next team discussion.
Quick answer
What does the workplace wellbeing self-check assess?
The FIT-UP self-check asks about goals, responsibilities, involvement, working conditions, access, privacy, communication, resources and evaluation. It groups your answers as clarified, partly clarified or open. For unresolved topics, it suggests planning tasks and relevant guides. It reviews project organization from your entries; it does not measure employee health or program effectiveness.
A discussion starting point for your wellbeing team
Answer from your current perspective. If you are unsure, choose “open or unknown”. An unresolved point initially needs discussion. The check does not assign grades or compare your company with others.
Discuss the overview with people involved in the plan, such as HR, project leads, employee representatives and relevant specialists. Different answers may reveal where responsibilities or information are not yet shared clearly.
Turn an unresolved point into a task
Choose one to three priorities and record what needs clarification, who will do it and by when. For example, if shift teams are rarely reached, an owner reviews accessible times and communication with those teams before choosing another format.
The check suggests an action for each unresolved question. Privacy, responsibilities and working conditions appear first when unclear. This order is an editorial planning aid, not a scientifically validated scoring model.
Repeat the questions after your discussion. Compare clarified tasks and record decisions in your strategy or annual plan. Store only the necessary organizational information, not personal health data.

Scope and limitations
- The questions and recommendations are an editorial FIT-UP self-check. They are not a validated audit, certification or proof of legal compliance.
- The check asks for organizational information. It does not collect personal health data or provide diagnoses.
- Your answers are evaluated in your browser and are not sent to FIT-UP. They are lost when you leave or reload the page unless you download the result yourself.
Frequently asked questions
Do I need to provide an email address?
No. Complete the check without registration, view the result and download it as a text file.
Is the self-check a wellbeing audit?
No. It structures your own information through editorial questions. It cannot replace professional assessment, certification or a review of legal requirements.
What does “open or unknown” mean?
You have not clarified the topic or cannot confidently assess its status. The result suggests a task for discussion.
Who sees my answers?
Evaluation happens in your browser. Answers are not sent to FIT-UP or stored permanently. If you download a text file, you decide how to keep and share it.
